NP - Home Based Primary Care (Healthcare)
As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients.
We are seeking a Nurse Practitioner to:
Provide appropriate direction to program and clinic staff regarding patient care issues.
Education:
Minimum –
If Nurse Practitioner: Master's or Doctorate's degree from accredited Nursing Program.
Work Experience:
Minimum – 2-3 years' experience
Preferred - Experience working in a Geriatric clinical practice as an independent clinician.
Licenses and Certifications:
Minimum -
If Nurse Practitioner: Active RN and APNP license in State of Wisconsin AND active board certification as a NP. These include the following: Adult; Adult/Gerontology Primary Care; Family; or Gerontology.
All Nurse Practitioner applicants must have: Active DEA license or ability to apply for such license prior to or upon hire. CPR certification and advanced life support certifications if relevant to practice area. Valid WI Driver's license and reliable vehicle.
Required Skills, Knowledge, and Abilities:
Physical Requirements:
Medium: Ability to lift up to 50 pounds maximum with frequent lifting/and or carrying objects weighing up to 25 pounds.
Work Schedule:
100% FTE, 5 days/week: Monday – Friday with various shifts within core hours of 8:00am to 5:00pm. Hours may vary based on the needs of the department.
