RN Coordinator (Project Management)
RESPONSIBILITIES AND DUTIES:

DEPARTMENTAL (JOB SPECIFIC TASKS):
Serves as a member of the IDT, as mandated by CMS regulations.Careplanning, initial, periodic, annual and change of condition assessments required under CMS regulations.CNA supervisory visits and documentation to meet the State Home Health Class A licensure requirements.Performs home visits to administer prescribed medications and treatments in accordance with nursing standards to supplement clinic services.Covers after hour and weekend call in rotation with other RM PACE NPs and RNs.Observes, records, and reports participants condition and reaction to drugs and treatments to physicians.Documents all interactions related to home care visits in the EMR.Instructs participants and family members regarding medications and treatment instructions in the home by CNAs, PCWs and homemakers.Keeps Home Care Manager and IDT informed on home care issues of participants as they arise.Posts advance directives in participants homes and educate participants and families regarding plans of care.Attends IDT meetings at care conferences, family meetings, and any other meetings as assigned.Maintains personal supplies and materials needed for home visits. Ensures cleanliness. Prepares list of medical supplies needed and in stock.Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.Other duties as requested/assigned.
QUALIFICATIONS AND REQUIREMENTS:
Graduate of an accredited Registered Nurse Program and licensed as a professional in the State of Colorado is required.Minimum of one year of health care experience in hospital, long-term or home health care setting is prefrered.CPR and First Aid certification is preferred.Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.Must submit to and pass a pre-employment drug/alcohol screening and criminal background check.Must have a current Colorado Drivers License and driving record must be in compliance with Company policy. Must maintain current automobile insurance.Proficient in software programs, such as Microsoft office and electronic medical records.Skilled in preparing and maintaining records, writing reports, and responding to correspondence.Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public.Must be free of communicable diseases that can be readily transmitted in the workplace. TB test is required within first 7 days of employment. Hep B vaccinations are offered but not required.Operates in full compliance with all applicable federal, state and local fire, health, safety, sanitation, Emergency procedures and other Standards prescribed in law or regulations.Knowledge of medical equipment and instruments.Skill in applying and modifying the principles, methods and techniques of practical nursing to provide on-going care for participants.Ability to perform phlebotomy functions.Skilled in identifying problems and recommending solutions.Skilled in developing and maintaining department quality assurance.Ability to react calmly and effectively in emergency situations.

Must have the ability to work under moderate pressure in order to meet scheduled appointments or deadlines while dealing with frail, disabled and/or confused participants/clients. Subject to individuals who may have the potential for physical or verbal aggression. May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and other conditions common in a clinic environment.
