We are looking for a Discharge Planner! The Discharge Planner assists Inpatient treatment team with discharge planning, coordinating out-patient appointment times with families and provides. this position attends treatment team meetings, providing guidance regarding discharge planning.
Bachelor's Degree
2 years' experience in the mental health field
