PEER MENTOR (Personal Services)



Peer Mentor

GENERAL RESPONSIBILITIES:   

  • Provide Peer Mentor services to persons assigned to caseload.
  • Mentoring through self-disclosure of personal recovery experience to assist with daily living skills, relationship building, identifying support networks and community resources, symptom self-management, and coping skills.
  • Assist in planning and implementing recovery group activities as assigned.
  • Work hours are flexible to meet the needs of the person served.
  • The majority of contacts happen out of the office and in community settings.
  • Contact with new referral is made within 48 hours of assignment.

ESSENTIAL JOB FUNCTIONS:

  • Will make all contacts as indicated in tx plan.
  • Weekly contact sheets turned in at the end of the workweek.
  • Progress notes are completed within 24 hours of service.
  • Will make regular contact with the treatment team.

MINIMUM QUALIFICATIONS: 

  • Must be at least 18 years of age and have a high school diploma or equivalent.
  • Must identify an individual who has been in sobriety recovery of a minimum of 1 year.
  • Excellent interpersonal and teamwork skills
  • Excellent organizational and time management skills
  • Interest and investment in working with adults who experience Severe and Persistent Mental Illness (SPMI) and their families.
  • Have completed or will complete any required training within six months of employment.
  • Must have a current and valid Kansas driver's license.
