RESPONSIBILITIES AND DUTIES:
ESSENTIAL JOB FUNCTIONS:
Schedules home care and clinic visits as requested.
Assists Care Coordinators with orders processing and tracking.
Answers and returns phone calls to participants, facilities, and outside providers.
Informs Care Coordinators and IDT of participant issues as they arise
Instructs participants and family members regarding test results, medication changes, and treatment instructions as instructed by the providers.
Attends IDT meetings at care conferences, family meetings, and any other meetings as assigned.
Serves as a scribe for the IDT and fulfills all scribe duties.
Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
Participates in Quality and Process Improvement meetings and activities as requested.
Documents all interactions with participants/MDPOA, facilities, and outside providers in the EMR.
Other duties as requested/assigned.

QUALIFICATIONS AND REQUIREMENTS:
Certified Medical Assistant from an accredited institution required
1 year experience working in an Electronic Health Record (EHR) required
Excellent writing and communication skills required
Experience working with the frail and elderly population preferred
Experience navigating scheduling systems preferred

Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to within normal range, with or without reasonable accommodation. Must be able to communicate effectively in verbal and written form with all levels of personnel within and outside of the organization.
Pressure Factor - Requires working under stressful conditions. Moderate pressure to meet scheduled and recurring deadlines.
Environmental Conditions - Working conditions may be noisy with fluctuating indoor temperatures. Must have the ability to work under moderate pressure in order to meet scheduled appointments or deadlines while dealing with frail, disabled and/or confused participants/clients. Subject to individuals who may have the potential for physical or verbal aggression. May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and other conditions common in a clinic environment. Subject to unpleasant odors.
