Description:
The case manager is an integral position of the treatment team. Case manager will work alongside the crisis clinician and our law enforcement partners in delivering service and providing resources to individuals and families experiencing mental health crises. While the case manager will provide short term case management services to any identified community member, the target population are those individuals 25 years and younger that have experienced a serious mental health crisis. The case manager is responsible for supporting the target population in linkage and connection to community and county-based services, providing psychoeducation, and providing emotional support/assistance to both the client and their family
How will I contribute
As part of the Metro CIT, case manager will be responsible for providing case management services to identified clients and their families. This can include referrals to both community and county-based services.Case manager is required to submit progress notes per Kingsview and DBH guidelines. Notes, per crisis guidelines, are required to be submitted within 24 hours of the service rendered.Provide psychoeducation to client and families.Ability to interact with law enforcement, community agencies and county-based services to obtain needed services for clients.Ability to work alongside clinicians in developing innovative ways to address identified barriers to treatment and resources.

Driving Requirements: A current CDL with an insurable driving recordEducation: 4-year degree preferredSpecialized Knowledge: Knowledge of local resources, community agencies and county-based services. Experience with documentation in EHR. Effective verbal and written communication. Comfortableness in providing services to seriously mentally ill individuals and their families.Characteristics: Great people skills and the ability to engage with a diverse population. Ability to work in a fast-paced environment.Experience: a minimum of 2 years experience providing case management services.
