Manager Care Management Are you looking for a new and exciting position working in healthcare? AbsoluteCARE, a Chronic Care Patient Centered Medical Home Center, is seeking a savvy clinical leader for the Full Time Manager Care Management. The Manager Care Management is responsible for handling a broad range of skills, healthcare expertise and proficiency in complex case management. The role will organize work groups, coach staff members, monitor performance and progress, enforce program standards, and ensure quality services. The role functions as a part of a large, integrated care team with the goal of providing whole-person care in the outpatient setting. The Manager Care Management will manage RN Care Manager team and Social Work team, in reference to the clinical team. Education & Experience Requirements: · Licensed Registered Nurse or Social Worker with 3 + years related Case Management Supervisory or Management experience required with experience in >2 care settings (hospital, SNF, home health, rehab) preferred. · Bachelor's degree with Case Management Certification a must, CCM preferred, · Knowledge and experience using Electronic Medical Records and ability to analyze and leverage their reporting capabilities is a significant advantage. · Handle confidential health care information following all HIPAA guidelines.
