QUALIFIED INTELLECTUAL DISABILITY PROFESSIONAL (Personal Services)
Essential Function
• Evaluation of consumer's strengths needs and develop Habilitation Plans, designed to develop personal and life skills.
• Ensure the timely receipt and filing of the Interdisciplinary Team summaries/ reports from various disciplines.
• Ensure timely completion of QIDP summaries for monthly, semi-annual and annual reviews. Submit semi-annual and annual reviews to Service Coordinator for inclusion in the individualized Service Plan.
• Complete addendum/changes to Residential Habilitation Plans as per team agreement.
• Coordinate consumer Residential Habilitation Services and collaborate with all clinical disciplines, which may include: Nursing, Speech/ Language, Nutrition, Psychology, Recreation, Medical, Pharmacy
• Notify team members, parents, guardians, relatives and day programs of all team meetings, reviews and apprise them of all habilitation changes affecting individuals.
• Ensure timely completion of all annual evaluations/ assessments by the various professional disciplines.
• Provide training to staff on Residential Habilitation Plans to ensure the appropriate implementation of plans.
• Participate in the development of Individualized Service Plan (ISP).
• May be assigned other tasks and duties by the Residence Manager that is reasonably related to their job responsibilities.
Skills and/or Experience Required
• Minimum of 3 years' experience working in OPWDD related field and preferably experience in a residential, health or treatment related setting preferred.
• Demonstrated competency in written, verbal and computational skills to document records in accordance with program standards.
• Should have the ability to work a flexible schedule if needed to accommodate working with the individuals in the residence and to attend team meetings.
