What You'll Do:

• Jump into Action: Evaluate new patients within 24 hours (or 1 business day) of physician referral. Can't meet the timeline? Just document the reason—it's that simple.
• Build Better Futures: Create effective, individualized treatment plans in collaboration with the referring physician, and help patients reach their full potential.
• Hands-On Healing: Deliver care according to the prescribed plan, keeping each patient's goals in focus.
• Lead & Inspire: Supervise Physical Therapy Assistants and collaborate with Restorative Aides to enhance care and share best practices.
• Collaborate for Success: Communicate openly with nursing, physicians, and other team members to discuss progress and overcome challenges.
• Stay Connected: Join team meetings like Care Conferences and Utilization Reviews to stay aligned and advocate for your patients.
• Educate & Empower: Guide families and caregivers in home programs to ensure smooth transitions from therapy to independence.
• Champion Recovery: Recommend equipment like walkers or grab bars to help patients live safely and independently.
• Stay Sharp: Document thoroughly, comply with regulations, and keep your skills up to date with training opportunities.

Who You Are:

• Licensed Physical Therapist in good standing.
• Graduate of an accredited Physical Therapy program (Bachelor's required, Master's or Doctorate preferred).
• Confident using clinical software like Casamba or PointClickCare.
• A great communicator who thrives in a team environment.
• Energetic, compassionate, and committed to patient-centered care.

Physical Requirements:

This is an active role—you'll be on your feet often, lifting, assisting, and working directly with patients. We'll support you with the tools and team to do it safely and effectively.

Pay: $58.00-$64.00 per hour depending on experience and education.
