Assess hospice patients to identify the physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations. 2. Implement/develop/document the plan of care to ensure quality and continuity of care. 3. Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. 4. Provide clinical directions to the Hospice Aide/LVN to ensure quality and continuity of service provided. 5. Assure continuity of quality patient care delivered with appropriate documentation. 6. Monitor assigned cases to ensure compliance with requirements of third party payor. 7. Demonstrate commitment, professional growth and competency. 8. Promote Agency philosophy and administrative policies. 9. Perform on-call responsibilities and provide on-call service to patients/families as assigned. 10. Provide effective communication to patient/family, team members, and other health care professionals
GENERAL FUNCTIONS:

Understand and comply with use and disclosure rules of resident Protected Health Information (PHI) as required by the HIPAA Privacy Rule of the Health Insurance Portability and Accountability Act. 7. Demonstrates leadership and problem-solving skills. 8. Supports mission statement, philosophies, and objectives promoting safe delivery of home health/hospice/PAS services. 9. Reports to work promptly as scheduled without absences. 10. Maintains a professional appearance and a business-like attitude. 11. Functions within the job description of his/her position. 12. Complies with agency policies set forth in the Code of Conduct and Employee Handbook. 13. Work cooperatively with all community members* and willingly accept assigned duties. 14. Report any possible problem or concern regarding community members* to Executive Director/Administrator immediately. 15.
